Elderly Services Specialist provides client-centered and trauma-informed services to support aging refugee populations aged 60+Services under this program are designed to foster independence, create pathways to community supportive services, and support citizenship for program participants. The Elderly Services Specialist will provide case management services, develop educational programming, coordinate socialization opportunities and assist clients in citizenship preparation. This is a full-time position which reports to the Director of Social Work. 

Job Overview:

Provide culturally sensitive and excellent case management, socialization, and education services to Refugee/Asylee/SIV clients aged 60 or older who have not yet received citizenship.

Duties:

  • Provide basic case management services
  • Engage with caregivers to support client outcomes
  • Assist with benefits applications, including but not limited to:
    • Supplemental Nutrition Assistance Program (SNAP)
    • Missouri Property Tax Credit (MOPTC),
    • LIHEAP, &
    • Metro Reduced Fare.
  • Coordinate quarterly meetings with education/service focus
  • Arrange for socialization activities to reduce loneliness and support integration
  • Refer eligible clients to immigration services to support citizenship
  • Provide information / education to assist enrolled clients in preparing for citizenship
  • Compile project data, analyze outcomes, and assist with supplying data for semi-annual reports
  • Document all services provided in a timely manner
  • Adhere to guidelines on mandated reporting
  • Perform other related duties and activities as appropriate

Qualifications

  • Bachelor's Degree in Social Work or Gerontology (BA/BSW) Preferred;  Associate Degree +2 years experience with elderly populations
  • Proficient in MS Office (Word/Excel/Outlook)
  • Experience utilizing Electronic Medical Record (EMR) systems
  • Excellent time management

  • Strong communication skills for use with limited English proficient speakers
  • Knowledge of community resources for elderly individuals
  • Knowledge of SSI benefits for elderly preferred
  • Knowledge of Medicare preferred
  • Knowledge of SNAP, MOPTC, LIHEAP, & Metro Reduced Fare systems preferred
  • Bilingual preferred. High level of language proficiency in Arabic, Farsi/Dari, Kinyarwanda, Nepali, Somali, Spanish, and/or Swahili preferred. Additional language proficiency in Amharic, Oromo, Tigrinya a plus.
